Qualifications

• Master’s degree in Social Work from CSWE-accredited program.
• Must hold licensed or certified social work at the advanced practice level.
• Two years post-advanced practice experience; one year at GS-11 level.
• Geriatric specialty experience preferred (CA LCSW).

Responsabilités

• Perform comprehensive psychosocial assessments for Veterans in GeriPACT services.
• Develop and review individualized treatment plans with Veterans and families.
• Provide counseling, psychotherapy, psychoeducation, and support groups.
• Coordinate referrals and community placements with VA and partners.
• Educate Veterans on VA benefits and long-term care options.
• Screen for suicide risk and health determinants; document in EHR.
