Job summary

The Institute for Family Health in Kingston, NY is seeking a Senior (Lead) Family Support Specialist to deliver in-home family support services under supervision of the Program Manager. You will build trusting relationships, establish case plans, coordinate with doctors and social service agencies, and help families access primary care.

This role requires a high school diploma or equivalent, 3 years of relevant experience, and a valid driver's license.

Qualifications

• High school diploma or equivalent required.
• Bachelor’s degree preferred; Master’s degree preferred.
• 3 years of relevant experience required.
• Valid Driver's License required.
• Familiarity with electronic health records (EHR) technology preferred.

Responsibilities

• Provides transportation to and from visits and trainings; mileage reimbursement per policy.
• Establishes trusting relationships with at-risk families, mainly in participants’ homes, with some evening hours.
• Establishes case plans with goals, objectives and activities with supervision.
• Assists parents with appointments and acts as liaison with agencies; ensures primary care access.
• Familiarizes self with community resources and how to access services.
• Meets with Supervisor weekly to review cases and status; attends meetings and trainings.
