Job summary

Bay Area Youth Services Inc. seeks a Lead Family Support Partner to provide intensive in-home therapeutic services that promote placement stabilization and support reunification efforts for families involved in child welfare.

The role requires a Master's degree in social work, at least 3 years of experience, and a valid Florida driver's license with auto insurance. Candidate should have experience with diverse populations and collaboration across service providers.

Qualifications

• Master's degree in social work or related area from an accredited college
• Minimum of three (3) years' experience in social work required
• Experience with juvenile justice or behavioral health systems preferred
• Experience navigating the child welfare system and mentoring families preferred
• Experience working with diverse populations and collaborating with service providers
• Must possess a valid Florida Driver's License and Auto Insurance

Responsibilities

• Document all case activities in the Florida Safe Families Network (FSFN) database within 48 hours
• Maintain the physical case file in chronological order, by subject, per standardized format
• Provide clients with timely referrals to services
• Develop knowledge of community resources and eligibility requirements, key contacts, and waiting lists
• Maintain regular contact with service providers and document progress in FSFN
• Follow Florida statutes, administrative code, and court orders in case management
• Attend and participate in individual case staffing as required; complete all staffing
• Conduct initial and ongoing child safety assessments as required
• Prepare initial and ongoing safety plans as necessary
• Attend all appointments, staff meetings, trainings, and related activities
• Collaborate with Program Directors, other Family Stabilization Specialists, and agency staff
• Ensure home visits and documentation are completed timely; attend court appearances as needed
• Take on additional tasks in the absence of the Program Director
• Perform other related duties as assigned
