Qualifications

• Bachelor’s Degree in Human Services or closely related field required.
• 3+ years’ experience with seniors or adults with disabilities may substitute for non-human services degree.
• Eligibility for long-term care functional screener certification is required

Responsibilities

• Manages an assigned caseload in collaboration with an RN Case Manager.
• Completes comprehensive member assessments and care plans.
• Provides ongoing care coordination and quality monitoring.
• Maintains required documentation per standards.
