Seven Hills Foundation is hiring a Direct Support Professional III to provide one-to-one support in a private home, helping individuals with disabilities build independence and engage in meaningful activities while partnering with caregivers.

You will implement ISPs, supports, and routines, assist with life skills and community activities, coordinate medical appointments, and document services with confidential records in line with agency standards.

Qualifications

• Must have a high school diploma or GED/HiSET.
• Driver’s license (Class C or D1) and 3 years of direct support experience.

Responsibilities

• Provide person-centered support in a private home and community.
• Implement ISPs, behavior supports, and therapeutic routines.
• Assist with life skills, household tasks, and community activities.
• Coordinate medical appointments and support overall health.
• Collaborate with families, caregivers, clinicians, and team members.
• Document services and maintain confidential records per agency standards.
