Seven Hills Foundation is seeking a Direct Support Professional III to provide one-to-one support in a private home and the community. You will implement individualized support plans, assist with life skills and household tasks, coordinate medical appointments, and collaborate with families and clinicians to promote health and independence.

Qualifications

• 3 years of experience in direct support, disability services, or related areas.
• High school diploma or equivalency required.
• Driver’s license and reliable transportation preferred.

Responsibilities

• Provide person-centered support in a private home and community.
• Implement Individual Support Plans and therapeutic routines.
• Assist with life skills, household responsibilities, and activities.
• Coordinate medical appointments and health/wellness support.
• Document services and maintain confidential records.
