Job summary

The State of Minnesota is hiring Direct Support Professional Senior in Forest Lake to assist individuals in short- and long-term housing programs. You will help clients achieve their best quality of life and personal growth across varied shifts.

Responsibilities include assessing needs, creating and updating support plans, documenting outcomes, and applying de-escalation and positive behavior strategies. Travel to appointments and activities is required; a valid driver’s license is essential.

Qualifications

• Experience with individuals with intellectual disabilities or mental illness.
• Experience with verbal de-escalation strategies and challenging behaviors.
• Clear communication with community members, employers and teams.
• Must be at least 18 years old and have basic math and computer skills.
• Must possess and maintain a valid Driver's License.

Responsibilities

• Assist in assessing individuals' needs through direct care duties.
• Develop and implement residential and habilitation support plans with goals.
• Evaluate outcomes and modify plans as needed.
• Respond using de-escalation strategies and positive behavior supports.

Skills

Experience with individuals with ID/MI

Verbal de-escalation techniques

Strong communication skills

Valid driver’s license

Ability to perform physical/essential

Education

CNA or PCA training

First Aid/CPR certification
