Seven Hills Community Services is seeking a Direct Support Professional II to empower individuals to live independently in their communities. You will provide daily care in a residential setting, teach life skills, and follow ISP/BSP guidelines to support health and well-being.

Responsibilities include medication assistance, documenting progress, mentoring staff, and building positive family and team relationships while maintaining a clean, safe home environment.

Qualifications

• High School Diploma or GED required.
• Must be at least 18 years old with an active US driver’s license.

Responsibilities

• Provide daily support in a residential setting.
• Teach life skills at home and in the community to foster independence.
• Follow Individual Support Plans (ISP) and Behavior Support Plans.
