Job summary

Senior Community Care is seeking a Registered Dietitian to join our interdisciplinary PACE team, supporting seniors living safely at home. You will assess nutritional status, develop individualized care plans, and provide evidence-based counseling to manage chronic conditions, maintain function, and prevent malnutrition.

You will collaborate with physicians, nurses, therapists, and social workers, monitor outcomes, and document in the EMR.

Qualifications

• RD credential required.
