Responsibilities

• Develop, coordinate, and monitor individual support plans for individuals with developmental needs.
• Provide case management and coordinate services across home, clinic, and community settings.
• Assess needs, plan interventions, and document progress and outcomes.
• Deliver crisis intervention and collaborate with clinicians, families, and providers.
