Job summary

The University of Louisville’s Trager Institute is seeking a Clinical Care Coordinator III to support older adults by coordinating case management, linking to community resources, and delivering dementia care training.

You will supervise social work students, engage in data collection with the team, and ensure high-quality care aligned with evidence-based practices. This role is grant-funded and located in Louisville.

Qualifications

• Associate's Degree in a related field with 2 years of relevant experience.
• Strong communication and advocacy for older adults.
• Experience with data collection and reporting preferred.

Responsibilities

• Coordinate case management services bridging clinical care and community resources.
• Provide support to older adults and families navigating healthcare systems.
• Advocate for the needs and rights of older adults within healthcare and community settings.
• Supervise and mentor social work students during field placements.
• Participate in data collection with the data team to ensure outcomes.
• Deliver training programs on dementia care for healthcare workers.
• Monitor and evaluate dementia care programs and adjust as needed for ADRD outcomes.
• Ensure programs and services meet relevant regulations and standards.
