Qualifications

• Minimum requirements include an Associate’s Degree in a related field and two years of relevant experience or equivalent.
• Excellent communication and interpersonal skills with ability to work in a multidisciplinary team.
• Experience with data collection and reporting is preferred and beneficial.

Responsibilities

• Coordinate case management services bridging clinical care and community resources.
• Support older adults and families navigating healthcare systems and accessing resources.
• Advocate for older adults within healthcare and community settings.
• Supervise and mentor social work students during field placements.
• Participate in data collection and collaborate with the data team on outcomes.
• Deliver training programs on dementia care for healthcare and supportive workers.
• Monitor and evaluate dementia care programs and adjust as needed.
