• Patient, Family, and Provider Collaboration: work with physicians, social workers, and other professionals to coordinate care.
• Interdisciplinary Communication and Team Support: prepare concise reports and support program goals.
• Professional Practice and Performance: participate in quality improvement to improve care transitions.
