Responsabilidades

• Provide outreach to clients 55+ and inform about resources
• Complete assessments to identify needs and services
• Coordinate resources and assist clients to meet needs
• Organize recreational, social and educational activities for seniors
• Engage, reassess and plan goals with clients
• Conduct well-being checks and respond to crises
• Develop senior groups to support community events
• Advise on community resources and track referrals and follow-ups
• Document in CMTS and meet reporting timelines
• Plan and coordinate activities at assigned buildings
