Job summary

Calderdale & Huddersfield NHS Foundation Trust seeks a Senior Rehabilitation Assistant to join a multidisciplinary team delivering home-based care. You will support adults of all ages who do not require acute hospital admission, helping them maintain independence within their own homes.

The role involves contributing to urgent community response initiatives, supporting discharge planning, and delivering mobility, exercise, nutrition and self-care guidance in line with service competencies.

Responsibilities

• Implementing treatment to protocols, and based on competencies.
• Monitoring progress and feeding back to the registered practitioner or assistant practitioner
• Making minor adjustments to treatment programmes within pre determined limits
• Assisting registered practitioners with complex treatment.
• May involve assessing for, ordering and issuing predetermined basic equipment to protocols and established guidelines.
• Act within the limits of your competence and authority i.e. work to competencies & local protocols and understand local communication networks.
• This includes making judgements around what may or may not be significant changes in a patient’s condition, involving observation and engagement skills and whether or not to report to a registered practitioner.
• Perform hand hygiene to prevent the spread of infection.
• Prepare individuals for healthcare activities and Support individuals undergoing healthcare activities as per service competencies.
• Monitor and manage the environment and resources during and after clinical/therapeutic activities including participation in infection control procedures when preparing and cleaning clinical areas, and undertake responsibility for stock control (including monitoring & ordering) of clinical & non clinical supplies.
• Assist the practitioner to implement healthcare activities e.g. assisting registered practitioners in the delivery of therapy programs, as per service competencies.
• Contribute to the discharge of an individual into the care of another service
• Provide advice and information to individuals on how to manage their own condition i.e. support patients with self-care and self-management of their condition.
• Assist in the implementation of mobility and movement programmes for individuals to restore optimum movement and functional Independence.
• Deliver exercise sessions to improve health and wellness
• Undertake personal hygiene for individuals unable to care for themselves
• Provide information and advice to individuals on eating to maintain optimum nutritional status
• Monitor and review individuals progress in relation to maintaining optimum nutritional status
• Collect, transport materials and equipment within the health sector
• Fit healthcare equipment, medical devices, assistive technology, or products to meet individuals' clinical needs
• Maintain healthcare equipment, medical devices and associated systems - ensure equipment is maintained in safe working order
