Job summary

Cambridgeshire and Peterborough NHS Foundation Trust is seeking a Senior Physiotherapist for the Intermediate Care Team. The role focuses on delivering high-quality, patient-centered rehabilitation in patients' homes, including care home settings, to support earlier hospital discharge.

You will work autonomously within an MDT, contribute to service development, and assist in training therapy assistants and integrated care workers to ensure safe patient flow and outcomes.

Qualifications

• Assess patients with complex and multiple pathologies using specialist clinical reasoning.
• Provide therapy interventions, identify patient needs, set goals and provide equipment and rehabilitation programmes.
• Work across Pathway 1 (care at home) and Pathway 2 (care home beds) for the Intermediate Care service.
• Autonomous practitioner within the integrated multi-professional team.
• Responsible for patients on shared caseloads in community and/or care home settings.
• Take a holistic approach to all therapeutic interventions, crossing professional boundaries when needed.

Responsibilities

• Keep the patient at the centre of care with patient and carer participation in decisions.
• Provide support and education to peers, junior staff and students.
• Be accountable for delegated tasks undertaken by Therapy Assistant Practitioners and Integrated Care Workers.
• Prioritise patients by clinical need and allocate tasks accordingly.
• Maintain a high standard of care and provide resilience to other locality teams during peak demands.
