Essex Partnership NHS Foundation Trust is seeking a Senior Community Nurse with special interest in Continuing Healthcare. The post focuses on coordinating CHC assessments and NHS Funded Health Care entitlements, in collaboration with the CHC team and local healthcare partners.

The role involves delivering high-quality community-based nursing care, working with General Practices, Social Services and other services to support health needs and independence, while contributing to out-of-hospital

Qualifications

• Registered nurse with valid NMC registration.
• Experience in community nursing or primary care.
• Ability to work autonomously and within multi-disciplinary teams.

Responsibilities

• Undertake comprehensive assessments to determine CHC eligibility.
• Lead and participate in care planning and delivery in community settings.
• Maintain accurate nursing records per NMC guidelines and local policy.
• Supervise teaching and placement for pre/post registration nurses.
• Contribute to guideline development and audits.
