Job summary

Job description

As a Senior Case Worker in the Hampshire County Council Continuing Healthcare (CHC) team, you'll work within a multidisciplinary environment collaboratively with your social care colleagues and other professionals in the varied and complex field of NHS CHC. You'll be supported to manage a diverse caseload, working across all care groups including older and younger adults, with mental health concerns and physical and learning disabilities. What you'll be doing:

• Supporting individuals with their onward care journey following a Discharge to Assess period from hospital.
• Undertaking assessments in line with the Care Act 2014.
• Engaging with multidisciplinary teams to support completion of NHS CHC assessments.
• Collaborating with individuals and their families to transition them to suitable care provision following assessment.
• Career growth: Our experienced Senior Case Workers can be considered for our social work apprenticeship scheme, leading to a career as a Qualified Social Worker. Knowledge of Care Act 2014 and it's eligibility criteria.
• Knowledge and understanding of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care July 2022 (Revised).
• Good verbal and written communication skills.
