Job summary

Medacs Healthcare is recruiting an experienced CHC Nurse Assessor (Band 6) to join the NHS Continuing Healthcare service on a locum basis. This fully remote role involves assessing care needs, managing a caseload, and coordinating packages of care across NHS and social care partners.

The ideal candidate will hold an NMC registration (RGN/RMN/RNLD), with CHC or related experience, and excellent communication skills to work with service users, families, and healthcare professionals.

Continuing Healthcare (CHC) Nurse Assessor - Band 6 (RGN/RMN/RNLD)

Current NMC registration as an RGN, RMN or RNLD, the Right to Work in the UK, and experience in Continuing Healthcare (CHC), community nursing, complex care, mental health, learning disabilities or clinical case management.

Job Summary

We're currently supporting Cheshire and Merseyside ICB in recruiting experienced CHC Nurse Assessors to join its Individual Assessment and Commissioning Team. This role would suit nurses with a background in Continuing Healthcare (CHC), community nursing, mental health, learning disabilities, complex care or clinical case management.

You'll assess care needs, manage a caseload and work with NHS services, local authorities and care providers to help individuals access the right care and support.

Who We're Looking For

You're an experienced nurse who enjoys assessment-based work, managing a caseload and working collaboratively with others to support positive outcomes. You'll be confident making informed clinical decisions and communicating with healthcare professionals, service users and families.

Key Responsibilities

• Complete Continuing Healthcare (CHC) assessments and reviews.

• Knowledge of the Mental Health Act, Mental Capacity Act or Section 117 pathways.
• Experience reviewing or commissioning care packages.
