Job summary

The McCall Behavioral Health Network is seeking a Senior Outreach & Engagement Case Manager to provide assessment and case management services to at-risk adults 55 and older in Western Connecticut. You will identify, engage, and refer seniors to community treatment options, education, counseling, and support services.

This full-time, non-exempt role requires strong communication skills, independence, and the ability to work in community settings.

Qualifications

• Bachelor’s Degree in Human Services or related field preferred.
• Proven clinical, verbal, and written communication skills.
• Ability to work independently and in the community.
• Spanish language skills preferred.

Responsibilities

• Assess clients by identifying strengths, needs, and goals for an individualized service plan.
• Develop service plans with clients, set goals, and determine recovery resources.
• Assist clients in gaining access to medical, social, educational, vocational, housing, and other essential services.
• Advocate for clients to ensure quality treatment and equitable care.
• Provide crisis prevention and early intervention to prevent relapse.
