UNIVERSITY OF OKLAHOM (Tulsa campus) is seeking a Senior Case Manager to support members with chronic and acute health needs through coordinated care and education. You will lead a diverse caseload, collaborate with clinicians, and mentor junior staff to ensure continuity of care and optimal outcomes.

The ideal candidate has a Bachelor’s degree in a health-related field, at least 2 years of case management/home-based services experience, and CCM certification or plan to obtain CCM within a year.

Job description

Pay Range:Targeted salary up to $50,400, depending on experience.

Work Schedule:Monday – Friday, 8:00 a.m. to 5:00 p.m.

Required Education and Experience:
  • Bachelor's degree in Social Work, Psychology, Sociology, or other Health or Human Services related discipline.
  • 24 months experience in home and community-based services, case management, or related healthcare setting.
Required Certifications and Licenses:
  • Certified Case Manager (CCM).The employee is allowed 1 year from hire date or promotion to Senior Case Manager to obtain the CCM certification.
Department Preferences:
  • One year of paid professional experience in direct service provision, care planning, and has provided services to the aging and/or disabled population in a home-like setting.
  • Case Management Certification upon hire or post hire.
