Job summary

Stamford Health in Stamford, CT, seeks a Senior Clinical Systems Analyst – Healthy Planet to design and optimize Epic Healthy Planet workflows that support population health management and care coordination. Qualifications

• Bachelor’s degree or equivalent is required.
• Minimum 3 years’ experience Epic support is required.
• Epic Analyst Certification or able to obtain within twelve (12) months of employment.
• Ability to work within a team environment.
• Excellent analytical, problem solving, written and verbal communications skills. Strong customer service skills.

Responsibilities

• Partner with Operational Care Coordination teams to design, implement, and optimize Epic Healthy Planet workflows that support population health management, care gap closure, patient outreach, transitional care, and longitudinal care coordination activities.
• Work closely with Operational Quality leadership and clinical stakeholders to support population health initiatives, quality improvement programs, regulatory reporting, and value-based care objectives.
• Designs, builds, configures, and maintains Epic modules.
• Identifies system issues proactively and ensures they are addressed before they affect business operations.
• Understands deep interdependencies between Healthy Planet workflows, upstream and downstream applications.
• Leads projects including new feature rollouts and workflow redesign.
• Develops test scripts and validates functionality during system upgrades.
• Completes required training and maintains Epic Certifications.
• Demonstrates the ability to work collaboratively with cross-functional internal teams and external partners to ensure alignment of workflows, system functionality, and operational goals.
• Leads Healthy Planet workgroups and joins other interdisciplinary/hospital committees to drive the use and application of electronic solutions.
