Delta-T Group is seeking a Clinical Supervisor Supports Coordinator in Dearborn, MI to manage person-centered plans and coordinate with families and providers. This contract role requires LMSW or LPC and a valid Michigan driver license, with a minimum of 3 years in mental health or related services.

You will coordinate with Skills Trainers and Employment Representatives, maintain documentation, and ensure compliance with consents and annual requirements.

Qualifications

• Minimum 3 years professional experience in mental health, developmental, or disability services.
• Proficiency in Microsoft Office.
• Valid Michigan Driver License.
• LMSW or LPC credential required.

Responsibilities

• Provide and monitor person-centered planning for an assigned caseload, including assessment of dreams/desires/needs and development of individualized plan.
• Complete monthly supports coordination contacts, clinical notes, and maintain case records in compliance with consents and physicals.
