Shift is Monday-Friday standard business hours. No nights, no weekends, and no holidays.
Travel is approximately 50% in Springfield, MA
This role supports the ACO REACH (Realizing Equity Access and Community Health) initiative, focusing on high-quality execution of transformation efforts and performance initiatives to drive success of the ACO model. The position involves collaborating with provider networks to improve care delivery, focusing on quality, patient experience, cost reduction, and profitable growth.
Responsibilities include analyzing provider performance data, supporting clinical performance meetings, identifying and addressing gaps in service, and understanding key drivers of value-based care such as clinical outcomes, growth, and financial performance.
Effective communication and relationship-building with organizational leadership are essential.
The typical pay range for this role is $87,035 - $187,500. The actual offer depends on experience, education, location, and other factors. This role is eligible for additional incentives such as bonuses or commissions.
