Job summary

Qualifications

• Experience: 2+ years in community health, harm reduction, or patient navigation.

Responsibilities

• Coordinate direct care and patient navigation for HIV, HCV, STI, and SUD programs.
• Lead targeted outreach, field-based engagement, and harm-reduction education.
• Maintain accurate HIPAA-compliant records across EMR systems and databases.
• Collaborate with clinical and behavioral health teams to ensure continuum of care.
