Job summary

WVU Medicine seeks a Clinical Pharmacy Specialist – Solid Organ Transplant to provide comprehensive pharmaceutical care for adult transplant patients across pre‑transplant to outpatient follow‑up. The role focuses on optimizing immunosuppressive regimens, managing drug interactions, and ensuring medication safety in a high‑acuity setting.

The position is a Monday–Friday day shift, on‑site, with no weekend hours.

Qualifications

• Current pharmacist license in WV, PA, MD, or OH as required by state of service.
• Basic Life Support certification within 60 days of hire.

Responsibilities

• Monitor and optimize drug therapy through rounds and clinical review.
• Lead and educate on safe, effective pharmacotherapy practices.
• Assist in discharge planning and patient education about medications.
• Provide drug information to medical and nursing staff and participate in quality initiatives.
• Mentor students and residents; engage in transplant program development and research.
