Qualifications

• Doctorate of Pharmacy (PharmD) or Bachelor degree in Pharmacy.
• Five years as a clinical pharmacist.
• Licensed Pharmacist in VA; BLS/ACLS within 90 days.

Responsibilities

• Interprets, prepares, and verifies medication orders.
• Provides clinical consultation and cost‑effective alternatives.
• Issues controlled substances in compliance with the DEA and VA Board of Pharmacy requirements.
• Participates in formulary review and approval processes.
• Monitors therapies for interactions, allergies, and appropriateness.
• Precepts learners and mentors residents/students.
