Job summary

Inova Alexandria Hospital is seeking a dedicated Clinical Pharmacist to join the team on a full-time basis. The shift varies and includes every other weekend. Sign-on bonus and relocation assistance are eligible for this role.

Qualifications

• PharmD or PharmD-equivalent degree required.
• Licensed Pharmacist licensed in Virginia.
• BLS/ACLS/PALS certifications within 90 days of hire.

Responsibilities

• Interprets, prepares, and verifies medication orders as appropriate.
• Provides clinical consultation and cost-effective alternatives.
• Issues controlled substances in compliance with DEA and VA Board of Pharmacy.
• Participates in formulary review and approval process.
• Monitors drug therapies for interactions, allergies, and appropriateness.
• Reports adverse drug reactions promptly and accurately.
• Minimizes non-formulary procurement and supports substitutions.
• Participates in continuous quality improvement.
• May perform additional duties including decentralised rounding with clinical teams, responding to emergent bedside alerts and codes, and precepting learners.

Education

Doctor of Pharmacy (PharmD) or Bachelor degree in Pharmacy

Licensed Pharmacist in VA

Clinical Pharmacist J ob Responsibilities:

• Participates in the medication formulary review and approval process.
• Monitors drug therapies for interactions, allergies, and appropriateness.
• Reports adverse drug reactions promptly and accurately.
• Minimizes non-formulary procurement and supports substitutions.
• Participates in continuous quality improvement.
• May perform additional duties as assigned, including, but not limited to:
  • Participating in decentralized rounding with clinical teams.
  • Responding to emergent bedside alerts and codes (adult and pediatric as designated).
  • Precepting learners as appropriate.

Minimum Qualifications:

Education: Doctor of Pharmacy (PharmD) or Bachelor degree in Pharmacy .

Licensed Pharmacist in the state of VA.

Certification: Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS) for Critical Care and Emergency Medicine, and Pediatric Advanced Life Support (PALS) within 90 days of hire.

Preferred Qualifications:

• Experience: One (1) year experience as a Clinical Pharmacist.
• PGY1 residency
• EPIC trained
