Qualifications

• PharmD or Bachelor in Pharmacy required.
• Licensed Pharmacist in VA.
• 5 years as a clinical Pharmacist.

Responsibilities

• Interpret, prepare and verify medication orders.
• Provide clinical consultation and cost-effective alternatives.
• Issue controlled substances per DEA/VA requirements.
• Participate in formulary review and approval.
• Monitor therapies for interactions, allergies and appropriateness.
• Report adverse drug reactions promptly.
• Minimize non-formulary procurement and support substitutions.
• Participate in decentralized rounding with clinical teams.
• Respond to emergent bedside alerts and codes in adult/pediatric settings.
• Precept learners; may lead grant work and education activities.
• Lead/participate in committees and regional/national professional activities.
