Job summary

Qualifications

• Must meet State requirements for Service Coordinator.
• Knowledge of IDEA, Part C Early Intervention Policies.
• Working knowledge of child development and family dynamics.
• Experience with developmental delays and at-risk populations.
• Experience writing outcome and qualitative notes.
• Experience with community support systems and resources.
• Experience with quality assurance.
• Effective, respectful communication with diverse populations.
• Strong problem-solving, mediation and teamwork abilities.
• Planning/Organizing skills; prioritizes and plans work efficiently.

Responsibilities

• Establish and maintain performance standards and metrics for LES case management.
• Intake and referral responsibilities and temporary case management coverage as needed.
• Coordinate evaluations, IFSP development and reviews with families and teams.
• Ensure services are family-centered, culturally responsive, and compliant with timelines.
• Maintain accurate documentation and data entry; collaborate with providers and community partners.
