Job summary

U.S. Department of Veterans Affairs - Veterans Health Administration in Martinez, CA seeks a Community Living Center Medical Director to lead clinical and administrative oversight of the CLC.

This role shapes rehabilitative, palliative, and hospice care for Veterans in a mission-driven setting. The CLC Medical Director serves as senior physician leader, delivering direct patient care, clinical supervision, and program leadership to ensure high-quality, person-centered, culturally competent care

Qualifications

• MD/DO degree from an accredited school.
• Unrestricted medical license in the United States.
• Board certification (or eligibility) in Internal Medicine, Family Medicine, or Ger iatric Medicine.
• Completion of an ACGME-accredited residency program.
• 3–5 years of clinical geriatrics/long-term care experience preferred.
• Prior leadership or medical director experience preferred.
• U.S. citizenship required for VHA physician positions.

Responsibilities

• Serve as attending physician and clinical leader for Veterans residing in the Community Living Center, overseeing admission, treatment planning, and discharge processes.
• Provide direct oversight of long-term care, short-term rehabilitation, respite, hospice, and palliative care services.
• Lead and collaborate with an interdisciplinary care team including nursing, social work, rehabilitation therapy, psychology, chaplaincy, dietetics, and pharmacy.
• Ensure compliance with CMS, Joint Commission, and VHA CLC regulatory and quality standards.
• Participate in quality improvement initiatives, infection control, and patient safety programs.
• Supervise and mentor resident physicians, fellows, and advanced practice providers as applicable.
• Serve as a liaison between the CLC and hospital leadership, GEC service line, and VA Central Office initiatives.
• Participate in family meetings, goals-of-care discussions, and advance care planning.
• Contribute to CLC staff education, competency development, and clinical protocol development.
• Engage in on-call coverage rotation as needed.
