VA Northern California Health Care System is seeking a Community Living Center Medical Director in Martinez, CA to lead clinical and administrative oversight of the CLC. You will guide long-term care, rehabilitative, hospice, and palliative services, while supervising an interdisciplinary team and ensuring regulatory compliance.

The role emphasizes patient-centered, elder-focused care with opportunities to mentor residents and collaborate with hospital leadership in a mission-driven environment.

Qualifications

• MD/DO degree from an accredited school.
• Current, full, unrestricted license to practice medicine in the U.S.
• Board certification (or eligibility) in Internal Medicine, Family Medicine, or Geriatric Medicine.
• Completion of an ACGME-accredited residency program.
• Minimum of 3-5 years of clinical experience in geriatrics, long-term care, or related setting preferred.
• Prior leadership, supervisory, or medical director experience strongly preferred.

Responsibilities

• Attend as the attending physician and clinical leader for Veterans in the Community Living Center, overseeing admission, treatment planning, and discharge processes.
• Provide direct oversight of long-term care, short-term rehabilitation, respite, hospice, and palliative care services.
• Lead and collaborate with an interdisciplinary care team including nursing, social work, rehabilitation therapy, psychology, chaplaincy, dietetics, and pharmacy.
• Ensure compliance with CMS, Joint Commission, and VHA CLC regulatory and quality standards.
• Participate in quality improvement initiatives, infection control, and patient safety programs.
• Supervise and mentor resident physicians, fellows, and advanced practice providers as applicable.
• Serve as a liaison between the CLC and hospital leadership, GEC service line, and VA Central Office initiatives.
• Participate in family meetings, goals-of-care discussions, and advance care planning.
• Contribute to CLC staff education, competency development, and clinical protocol development.
• Engage in on-call coverage rotation as needed.
