Qualifications

• High school diploma plus six years of related field experience (education may substitute).
• Experience with CHW procedures and scope of practice.

Responsibilities

• Establish close working relationships with patients and interprofessional team to identify non-medical drivers of health and reduce health disparities.
• Promote goal setting and strategic planning for overall wellness.
• Accept referrals and provide case management for CPW and chronic disease management for IPCP.
• Document case management activities in the electronic health record.
