JOB SUMMARY:

The Community Health Worker (CHW) for AccessHealth will assist an assigned group of patients to maintain a Primary Care Medical Home, as well as increasing their access to services within AccessHealths continuum of care and/or the community at large. The CHW will link patients with resources available to them within the community. The CHW for AccessHealth is a frontline public health worker who serves as a liaison and link between the company and health/social services.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
  • Screens all referrals received from outside entities to determine which program a patient is best suitable for.
  • Assist assigned population with accessing resources, including, but not limited to food, health insurance, housing, quality care, transportation, and health information.
  • Follow up with assigned group of patients to ensure access to medical care.
  • Link clients with appropriate resources and referrals to outside agencies.
  • Assesses needs of clients and perform intakes.
  • Meet with client in the most appropriate setting- clinic, home, hospital.
  • Participates in department meetings and appropriate meetings with outside entities as determined by management.
  • Properly documents activities performed and assistance provided in the clients chart in a timely manner.
  • Prepares weekly client reports and maintains program rosters.
  • Updates management and others as appropriate of any changes to the program roster.
  • Contacts patients between visits to follow up on status.
  • Assist an assigned group of patients in applying for Medicaid, CHIP, and CHIP Perinatal programs, PHC, Food Stamps and Temporary Assistance to Needy Families.
  • Develop a system for determining eligibility expiration for the assigned population, assist current patients to continue their eligibility, and initiate registration process when needed.
  • Accepts job related training and development in new tasks and technologies.
  • Performs home visits as deemed necessary by management.
  • Comply with all company policies, procedures and regulatory requirements.
  • Performs all other duties as assigned by management.
EDUCATION/LICENSURE/CERTIFICATION QUALIFICATIONS:
  • 1. High School Diploma or GED.
  • 2. Certified Community Health Worker or two years working in a community health center.
  • 3. Other experience may be accepted in lieu of Education requirements at the discretion of management.
SKILLS AND EXPERIENCE:
REQUIRED:
  • 1. Two years of experience in customer service.
  • 2. Ability to operate a computer, fax machine, and scanner.
  • 3. Good communication skills.
  • 4. Ability to work with people of all socioeconomic levels
  • 5. Ability to communicate orally and in writing with good organizational skills.
  • 6. Be a consumer advocate for quality care and services.
PREFERRED:
  • 1. Experience with Medicaid, Medicare, and the Healthcare Marketplace and CHIP insurance programs.
  • 2. Knowledge of community resources.
  • 3. Bilingual ability preferred.

HOURS:

Employees will be assigned to a regular work schedule, which may include hours before 8:00 am, after 5:00 pm, and Saturdays as determined by department needs and requirements.

QualificationsQualifications / Skills & Experience
  • Bilingual (English/Spanish) preferred.
  • Experience in community outreach, social services, resiliency, advocacy, or disaster recovery preferred.
  • Ability to work both independently and collaboratively within a team.
  • Experience with case management or client file management.
  • Intermediate proficiency in Microsoft Office (Word, Excel, Outlook) and other systems.
  • Professional and effective communication skills when interacting with clients and staff.
Education Requirements
  • Associate's or Bachelor's Degree required (preferred in Public Health, Social Work, or related field).
