Job summary

ChenMed in Memphis is seeking a Primary Care Physician to serve seniors in a value-based care model. You will manage a patient panel of up to 450 seniors and lead a care team focused on proactive care and health coaching for Medicare Advantage patients.

Required are an MD or DO in Internal Medicine, Family Medicine, or Geriatrics, with active state licensure and a DEA license; board certification is preferred and BLS is required. This role emphasizes quality improvement and team collaboration.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, or Geriatrics.
• Minimum of 2 years of clinical experience.
• Active state medical license and DEA license.
• Board certification preferred; BLS certification required.

Responsibilities

• Diagnose and treat acute and chronic illnesses for a panel of up to 450 seniors.
• Provide health coaching to help patients set and achieve health goals.
• Lead a care team including promoters, facilitators, and coordinators.
• Collaborate with hospitalists, specialists, and transitional care teams.
• Be accountable for quality metrics and participate in clinic leadership and quality improvement.
