Qualifications

• Master’s degree in Social Work.
• At least two years’ experience in providing social services.
• Experience in providing case management or social services within the senior population.

Responsibilities

• Advocate for clients to resolve challenges.
• Collaborate with other professionals to evaluate clients’ medical or physical conditions and assess their needs.
• Refer clients or families to community resources to assist in recovery from mental and physical illness and to provide access to services such as financial assistance, legal aid, housing, education, benefits, entitlement and job placement.
• Counsel clients in individual and group sessions to help them improve quality of life.
• Organize support groups on topics such as anxiety, bereavement, depression, etc.
• Organize support groups or counsels for family members to assist in understanding and supporting clients.
• Maintain written records of contact with clients, including case files, worker logs, and computerized client databases.
• Attend required training, including mandatory funding organizations.
• Assist agency in outreach activities in the community.
• Abide by all HIPAA policies and procedures.
• Other duties as required by the Director.
