Job summary

Catholic Charities is seeking a self-motivated leader who is dedicated and detail-oriented, compassionate, trauma-informed, and culturally aware to serve as a Case Manager III in our Aging and Disability Services.

This role works directly with assigned clients to develop the client's service plan, including intake and annual assessments, available community partners and resources, and applicable Catholic Charities services.

Qualifications

• Minimum of a Bachelor’s or Master’s degree in social work, psychology, sociology, human services, or related field preferred.
• Experience with diverse populations and communities is required.
• Ability to work in a fast-paced, team-oriented environment and manage complex cases.

Responsibilities

• Conduct initial and annual assessments and develop service plans for clients.
• Coordinate with community resources and Catholic Charities programs to meet client needs.
• Advocate for clients and maintain complete case documentation and records.
• Support education and outreach related to services and challenges.
