Qualifications

• Bachelor’s Degree in Social Work or related human service field.
• Experience in case management and budgeting.

• Knowledge of HIPAA and client recordkeeping is essential.

Responsibilities

• Maintain HIPAA-compliant case files and intake documents.
• Coordinate program activities and assess client eligibility with the Clinical Services Team.
• Assist clients with goal setting and identifying their needs.
• Produce timely reports and monitor service goals.
• Work with agencies and volunteers to coordinate services.
