Encore Community Services in Midtown Manhattan is seeking a Case Manager to provide in-home assessments and comprehensive case management for homebound older adults under the Expanded In-Home Services for the Elderly Program (EISEP). The role includes coordination with homecare providers and advocacy to ensure needs are met.

The position is full-time, on-site with one work-from-home day after probation, offering benefits such as medical, dental, vision, PTO, and retirement plan options.

Qualifications

• Master’s Degree in Social Work preferred or a Bachelor’s Degree in Social Work or related field with alternative qualifications.
• At least 3 years of professional social work experience with an elderly population.
• Bilingual (Spanish) or Mandarin/Cantonese preferred.

Responsabilités

• Perform in-home assessments and reassessments of homebound older adults and develop comprehensive care plans.
• Provide ongoing case management services including information, counseling, referral, and crisis intervention.
• Keep records of all client contacts and maintain case files in the database.
• Coordinate referrals with homebound meal and homecare agencies.
• Ensure compliance with regulatory and programmatic requirements; document contacts and visits.
• Attend mandatory training and perform related duties as assigned.
