Job summary

The Pennant Group, Inc. is seeking an Advantage Case Manager in Edmond, OK to oversee case management for aging or disabled populations. You will work under CMS management, manage intake, assessment, service plans, and discharge while coordinating resources and ensuring regulatory compliance.

Qualifications include RN or LPN with CACM or a Bachelor's degree with CACM and one year of relevant experience. A valid driver’s license and good driving record are required.

Qualifications

• Must hold RN license or LPN license with 1 year of experience and CACM credential
• Bachelor’s degree with 1 year of experience and CACM credential
• Must be a licensed driver with insured vehicle and good driving record
• Fluent English with excellent verbal and written communication skills
• Intermediate computer skills using office software

Responsibilities

• Establish intake, screening, and referral procedures for community long-term care options.
• Review assessments and reassess as needed to identify member needs and goals.
• Develop and monitor service plans with interdisciplinary team.
• Coordinate services and community resources for members.
• Document and submit plans within required timeframes.
• Ensure confidentiality and HIPAA compliance.
• Provide education to empower members and act as advocate.
