Job summary

The Pennant Group, Inc. is seeking an Advantage Case Manager in Tulsa, OK, to manage core case management activities for aging and disabled populations under OKDHS MSU guidelines.

Qualifications

• RN/LPN/Baccalaureate with required experience and Certified Advantage Case Manager credential.
• Must be licensed driver with insured automobile.

Responsibilities

• Carry out the case management process including admission/intake, assessment, service plan implementation, monitoring and discharge.
• Establish intake, screening, and referral procedures for information requests about community long-term care options.
• Review assessments to identify member needs, goals, abilities, resources, and support for service planning.
• Orient and educate consumers to empower informed and active participation.
• Coordinate available community resources and informal support.
• Submit comprehensive written service plans per IDT process and monitor outcomes.
• Identify and report abuse, neglect or exploitation as needed.
• Ensure confidentiality and HIPAA compliance.
