Job summary

The Pennant Group is seeking an Advantage Case Manager in Edmond, OK, to oversee intake, assessment, and service planning for aging or disabled members. The role emphasizes collaboration with the IDT, coordination of community resources, and ongoing education for clients.

Qualifications

  • RN or LPN licensure with relevant aging/disabled experience and CCM certification required.
  • Baccalaureate Degree with relevant case management experience and CCM preferred.
  • Fluent English, strong written and verbal communication skills.
  • Must have reliable transportation and valid driver’s license.
  • Intermediate computer skills and standard office software proficiency.

Responsibilities

  • Carry out intake, assessment, service plan implementation, monitoring, and discharge per MSU/OKDHS guidelines.
  • Develop and document comprehensive service plans via IDT process with measurable goals.
  • Coordination of community resources and support to promote independence and cost efficiency.
  • Monitor service delivery, evaluate outcomes, and update plans as needed; report issues promptly.
  • Maintain HIPAA compliance and confidentiality across all records; ensure timely documentation.
  • Participate in training, CQI processes, and provide mentorship to junior case managers.
