Job summary

The Pennant Group, Inc. is seeking an Advantage Case Manager to perform case management per Oklahoma DHS MSU standards, under CMS/Excell supervision. Duties include intake, assessment, service plan implementation, monitoring, and discharge of members.

This exempt role requires coordinating with the IDT, timely documentation, member education, HIPAA compliance, and coordination of community resources. Local travel may be required, with emphasis on ethical care and accountability.

Qualifications

• Minimum qualifications include RN or LPN with 1 year of paid professional experience with aging or disabled population and Certified Advantage Case Manager.
• Must be a licensed driver with a vehicle.
• Fluent English with excellent verbal and written communication.
• Must manage time effectively amid fluctuating caseloads and coordinate with others.

Responsibilities

• Establish intake, screening, and referral procedures for information requests about long-term care options.
• Review assessments to identify member needs, goals, and plans.
• Submit comprehensive service plans and document timely.
• Coordinate with interdisciplinary teams to implement plans and monitor outcomes.
