Job summary

ACMH INC in Queens (Jamaica), New York, is seeking a Senior Case Manager/Case Manager for Markus Gardens SP-SRO. The role involves coordinating care and delivering community rehabilitation services within a team-based framework.

Position includes regular home visits, psychosocial assessments, and close collaboration with psychiatric care providers. Scheduling covers Tue–Sat shifts; reports to the Program Director and includes housing support and medication management elements.

Qualifications

• M.A. in psychology or a human service related field (Senior Case Manager).
• B.A. in psychology or a human service related field with at least 3 years of direct care experience.
• Experience working with adults with serious mental illness preferred.

Responsibilities

• Conducts home visits for assigned caseload.
• Obtains psychosocial histories and conducts functional skills assessments.
• Maintains liaison with primary psychiatric care providers.
• Ensures access to psychiatric, substance use, medical services and entitlements.
• Monitors medications, adherence, and side effects.
• Develops individualized Service/Support Plans with the team and client.
• Delivers housing support, education/employment assistance, health care and entitlements.
• Documents client progress and updates plans.
• Trains clients in safety procedures and conducts drills.
• Attends in-service training and provides coverage for others as needed.
• Acts as Qualified Mental Health Staff Person.
