Job summary

TASC (Treatment Alternatives for Stronger Communities) is seeking a compassionate Case Manager to support clients involved in the criminal legal system who struggle with substance use and behavioral health challenges. You will conduct assessments, referrals, and case management to promote recovery and long-term stability.

This role requires collaboration with partners, adherence to DSM-5 and ASAM criteria, and documentation in required systems.

Qualifications

• High school diploma or GED required.
• Valid driver's license required.
• CADC certification within two years of employment may be required.
• Pass IDOC background check.
• Proficiency with Microsoft Office applications.
• Experience facilitating groups.
• Familiarity with ASAM criteria, DSM-V and professional ethics.

Responsibilities

• Conduct comprehensive assessments and develop service plans.
• Facilitate intake and enrollment into TASC programs.
• Provide referrals and crisis intervention for behavioral health, housing, employment, and other supports.
• Collaborate with community partners and internal staff for coordinated care.
• Apply DSM-5 and ASAM criteria in evaluating client needs and treatment referrals.
• Maintain timely documentation, progress notes, and discharge summaries.
• Monitor progress toward service goals and reporting compliance.
• Participate in staff meetings, trainings, and multidisciplinary discussions.
• Support clients in navigating systems and accessing essential services.
