Job summary

ACMH seeks a Senior Case Manager/Assistant Program Director for Queens Treatment Apartment (QTA). The role coordinates case management and assists the Program Director in operations, reporting to the Program Director, with location in Queens and a Mon–Fri 9:00am–5:00pm schedule.

The position requires a master’s degree in psychology (or bachelor’s with significant experience) and experience in mental health residential settings.

Qualifications

• Master’s degree in psychology or related field is required.
• Bachelor’s degree in psychology or related field with significant experience is acceptable.
• Understanding of evidence-based practices in rehabilitation and recovery.

Responsibilities

• Conducts home visits for assigned caseload.
• Obtains psychosocial histories and conducts functional skills assessments.
• Establishes liaison with psychiatric care providers.
• Ensures access to psychiatric, substance abuse and medical services and entitlements.
• Monitors client medication adherence and side effects.
• Develops and updates individualized rehabilitation plans with the team.
• Delivers community rehabilitation services per client need.
• Documents client progress and updates the Service/Support Plan.
• Trains clients on safety procedures and conducts drills.
• Prepares residents for transition to independent living and referrals.
• Provides crisis intervention when needed.
• Staffs Emergency Phone on a rotating basis.
• Maintains charts and submits reports per agency procedures.
• Oversees maintenance of apartments and alerts for issues.
• Acts as Qualified Mental Health Staff Person.
• Ensures service delivery complies with NYS OMH regulations.
• Supervises on-duty in director’s absence.
• Performs supervisory, program management and QA duties.
