Job summary

Little Friends, Inc. in Warrenville, IL is seeking a Case Manager for the adult residential program to coordinate and monitor services for individuals served under DHS standards and Little Friends’ policies. The role leads program staff and ensures Personal Plans are implemented with team collaboration.

You will supervise staff, provide CBTA/OJT training, and maintain documentation and compliance. Hours are Mon-Fri 9am-5pm, 37.5 hours/week, salary $50,000-$55,000 with on-call stipend.

Qualifications

• Must meet DHS criteria.
• Must be at least 21 years of age.
• Minimum of one (1) year experience working with individuals with developmental disabilities, preferably in a CILA.
• Must have a valid State of Illinois Driver’s License, insurance, and have a good driving record.

Responsibilities

• Maintain knowledge of Little Friends, Inc. mission, values, and goals and embed them in the Residential Program.
• Monitor programming for each individual served, including goals, choice-making, and progress.
• Ensure each individual has an implementation strategy per Personal Plan and team.
• Provide CBTA/OJT training and support to staff.
• Maintain records and documents per DHS standards.
• Establish and maintain communication with individuals served, guardians, and other providers.
