Job summary

Bethesda-Project in Philadelphia, PA is seeking a Senior Case Manager to work full-time on site. The role involves directly engaging with guests, assessing needs, and coordinating housing plans to stabilize medical and mental health conditions, guiding residents toward independence.

The position reports to the Program Coordinator and requires a bachelor’s degree in social work or related field, with master’s preferred, and experience in homelessness and recovery programs.

Qualifications

• Bachelor’s Degree in Social Work or a related field required. Master ’s Degree preferred.
• Experience in case management, strong understanding of homelessness, substance abuse and mental health issues.
• Knowledge of the recovery process and drug and alcohol inpatient and outpatient programs.

Responsibilities

• Provide an assessment of each guest’s needs and oversee the creation of an individualized Housing Plan.
• Demonstrate a thorough understanding of the Safe Haven Model and the application of Harm Reduction in all guest interactions and Housing Plan interventions.
• Conduct Shelter Intake Interviews with potential candidates.
• Meet with the CSP Program Coordinator and shelter staff (formally and informally) to identify guests in need of case management services.
• Assist guests in maintaining important relationships and serve as an advocate with service providers.

Salary Range: $25.00 To $27.00 Hourly

The Senior Case Manager engages directly with guests, assesses needs, makes appropriate referrals or provides supportive services to stabilize medical and mental health conditions in order to prepare residents for more independent living.

Physical Requirements:

Requires repetitive movement of hands and fingers for typing and/or writing. Frequent standing, sitting, walking, climbing stairs, occasional stooping, kneeling, or crouching and lifting up to 20 lbs. Ability to reach with hands and arms and perform audio and visual assessments, as required.
