Job summary

Qualifications

• Five years of experience supervising Case Managers in transitional/shelter programs or related setting.
• Associates degree in Human Services with 2–3 years supervising Case Managers.
• Bachelor’s Degree in Human Services with 1–2 years supervising Case Managers.
• Working knowledge of substance abuse treatment modalities.
• Available to work a flexible schedule as needed.
• Experience working with diverse populations.

Responsibilities

• Coordinate, guide, and supervise activities of direct reporting staff.
• Provide direct supervision of Case Managers.
• Communicate all aspects of contract requirements to staff.
• Lead and participate in administrative and staff meetings as required.
• Troubleshoot problems and make decisions per program policies.
• Monitor clients’ progress and document via progress notes.
• Conduct chart auditing of client files for accuracy and completeness.
• Directly manage a case portfolio of ~25 clients and maintain a therapeutic milieu.
• Provide individual counseling and facilitation of caseload group meetings.
• Provide psycho-education and counseling related to substance use and relapse prevention.
• Administer appropriate behavioral interventions as needed.
• Participate in assessment screenings and plan discharge/continuing care.
• Attend case conferences and other meetings as scheduled.

The Role

Under the general direction of the Assistant Program Director, the Senior Case Manager is responsible as a senior person for ensuring the smooth operation and coordination of all case management services following all program goals, targets, and performance outcomes. Provides case management, and advocacy services to an assigned caseload and participates as a member of an interdisciplinary treatment team to facilitate the client recovery/rehabilitation process.

Responsibilities

What You Will Do

• Coordinates, guides, and supervises activities of direct reporting staff.


• Provides direct supervision of Case Managers.


• Communicates all aspects of contract requirements to staff.


• Leads and participates in administrative and staff meetings as required.


• Troubleshoots problems and makes decisions in accordance with program policies, procedures, and protocols.


• Monitors clients’ progress toward their goals and documents via progress notes.


• Conducts periodic reviews of all case documentation to ensure accuracy, completeness, and clarity.


• Conducts chart auditing of client files.


• Directly manages a case portfolio of approximately 25 clients. Participates in monitoring therapeutic environment to ensure that behavioral guidelines are followed, a therapeutic milieu is maintained and interpersonal relationships remain positive.


• Provides individual counseling and facilitation of caseload group meetings and other assigned groups.


• Provides psycho-education and counseling germane to chemical dependency and relapse prevention.


• Administers appropriate behavioral interventions as needed.


• Participates in assessment screenings to determine client needs/preferences; formulates and implements treatment, transition, and discharge/continuing care planning collaboratively with individual clients.


• Attends case conferences and other meetings as scheduled.

Qualifications

Who You Will Be

• Someone with a High School Diploma or Equivalent with Five (5) years of experience preferably supervising Case Managers within transitional/shelter programs or related setting

OR

• Associates Degree in Human Services or related field with at least Two-Three (2-3) years of experience supervising Case Managers within transitional/shelter programs or related settings

OR

• Bachelor’s Degree in Human Services with at least One-Two (1-2) years of experience supervising Case Managers within transitional/shelter programs or related settings.


• Working knowledge of substance abuse treatment modalities and client self-help/support modalities.


• Available to work a flexible schedule as needed in response to participant and staff needs.


• Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds.
