Job summary

Prime Healthcare's St. Mary's Hospital in Kankakee seeks a Working Manager of Case Management to lead a comprehensive program and drive quality outcomes. You will mentor staff, oversee discharge planning, and coordinate utilization review to ensure medical necessity and cost efficiency.

Selected candidate will bring strong clinical credentials, at least 5 years in case management, and proven leadership in hospital settings. Excellent communication and analytical skills are essential.

Qualifications

• Licensed clinician in your state is required.
• MSW/LCSW with 5+ years post-graduate experience is required.
• Minimum 5 years’ experience in a Case Management position.
• Analytical ability for problem identification and evaluation of data/statistics from ongoing reviews.
• Basic to intermediate computer skills.

Responsibilities

• Develop staff and systems to operate a comprehensive Case Management Program.
• Provide leadership and supervision to case managers, social workers, discharge planners, and utilization review staff.
• Assess needs and plan services aligned with hospital mission and patient/family needs.
• Coordinate services using quality improvement tools to ensure clinical and cost-effective outcomes.
• Perform holistic admission and concurrent reviews of medical records for medical necessity and severity of illness.
